Welcome to the South Hadley Police Department's on-line crime reporting system! If you have an EMERGENCY, or the crime you want to report is happening RIGHT NOW, do NOT use this form; dial "911" on your telephone (this on line reporting system is not monitored continuously). If you need to report a minor crime involving the theft of property, minor personal injury, grafitti, etc., please complete and send us this electronic crime report.

INSTRUCTIONS:

1) This on-line report form should only be used for the crimes indicated in the check boxes appearing after these instructions. If it doesn't fit into one of those catagories, then you should report the crime in person or by telephone at 538-8231. STOLEN VEHICLES and MISSING PERSONS cannot be reported on this form.

2) Please use this form only to report crimes that occurred in South Hadley.

3) Please complete as many lines of this report as you possibly can. This will help us investigate the crime that you're reporting and screen it for potential investigation. Whenever you have only partial information (like part of a vehicle license plate number, for example) please give us whatever you have!

4) If you need to report more than one type of crime, or multiple instances, please send us a separate Report Form for each incident and/or type of crime.

IMPORTANT NOTE: Knowingly submitting a false Crime Report is a misdemeanor under the Massachusetts State Laws Chapter 269 Section 13A (and worth up to a year in jail) !! "Don't even THINK about it"
